TVS application and agreement form must be completed
and returned to TVS before any searches and services can
be performed. A valid signature is required; therefore, the
application and agreement form cannot be completed
electronically nor submitted via e-mail.
To apply for searches and services, please follow these steps:
1. Access the TVS Customer Application and Agreement form
by clicking here.
2. Print a hard copy of the form using the "print" function
of your browser.
3. After you have read the "Terms and Conditions" section
of the form, fill in all required information in both the top
and bottom sections.
4. Affix your signature to the Application and Agreement.
5. Return the form by faxing it to TVS at 412-798-0796; or,
